• REGISTRATION APPLICATION
A completed registration form is required for each student. It is recommended that you submit your application online at least one month before the class begins, as registrations are processed on a first-come, first-served basis. Please visit: www.clinicalsoftwareservices.com for updated course descriptions, availability, and pricing. You will receive an acknowledgment email within 2 business days of receipt.
• WAITING LIST
In the event that a class is full, a waiting list will be made available. Customers will be placed on the waiting list on a first-come, first-served basis. Customers on the waiting list will be contacted as soon as possible should space become available, and will be given the option to register.
An invoice for payment will be sent within 4 business days of registration. Payment must be received 10 days prior to the start of the class, unless other arrangements are made. Payments not received within 5 business days prior to the start of the class may result in cancellation of registration, and the space made available to another attendee. Payment can be made by check made out to Clinical Software Services, or by credit card.
• PRE—PAYMENT through GE
Customers who have pre-paid for Visual Form Editor classes through GE, or have included Visual Form Editor classes on their GE contract, must supply the GE PO number prior to the first day of the class. If a GE PO number is not received prior to the first day of class registration will be subject to cancelation until a PO number is received. If payment is not received through GE or GE disputes payment for the class, the business entity of the registering student will be responsible for the full cost of the course.
• TRANSFERS AND CANCELLATIONS
Cancellations must be received at least 10 days prior to the start of the class to receive a refund. In the event of a cancellation received after the cancellation period, a refund will be granted only if the space can be filled by another student from a waiting list. No-Shows or partial attendance will not be granted a refund. Course registration may be transferred to another attendee from the same site at any time with proper advance notification.
• CLASS SCHEDULE CHANGES AND CANCELLATIONS
It is sometimes necessary to change the dates a class is offered or to cancel a class. Registrants are contacted at the earliest opportunity in the event of a scheduling change or cancellation.
• TRAVEL, LODGING & MEALS
Registrants are responsible for making their own lodging arrangements. Transportation to the training site is the registrant’s responsibility. Clinical Software Services assumes no responsibility for nonrefundable travel arrangement losses resulting from course scheduling changes or cancellations. Breakfast and Lunch is provided with most classes, Dinners and other meal expenses are the responsibility or the registrant.
• COURSE MATERIALS AND EQUIPMENT
Each course attendee must provide their own laptop and software. Laptops should be of sufficient quality and speed to run the required software. Clinical Software Services will provide all other course materials, as well as extension cords and power strips to provide electricity. Clinical Software Services assumes no responsibility for loss, theft or damage to any equipment that may occur during the class, or in transit to and from the class site.
